WHEN THIS CONSULTATION MAY BE APPROPRIATE
A consultation may be considered when:
An individual wants contemporaneous documentation of current cognitive functioning
An attorney or estate-planning professional recommends additional clinical information
Family members have noticed changes in memory or day-to-day functioning
Significant estate or financial planning decisions are underway
A physician or other healthcare professional believes additional cognitive information would be useful

Relevant History
Medical, psychiatric, functional, and other history relevant to the clinical question.
Clinical Interview
A structured telehealth discussion of current concerns, functioning, and context.
Objective Cognitive Measures
Clinically appropriate digital or performance-based measures.
Collateral Information
Information from family members, caregivers, physicians, or other appropriate individuals when relevant and authorized.
Integrated Interpretation
Multiple sources of information are considered together rather than relying on a single score or measure.

Priority Access

For time-sensitive professional or personal planning needs.
Final report delivered within 7–10 business days following completion of the clinical consultation and receipt of all required assessment materials.
Standard Access
$750
Our comprehensive consultation pathway for individuals who do not require expedited reporting.
Final report delivered within 21–28 business days following completion of the clinical consultation and receipt of all required assessment materials.
The same consultation process and reporting standards apply to both access options.
WHAT YOU RECEIVE
A professionally documented consultation report.
The report may include the referral question, relevant history, assessment findings, clinical impressions, limitations, and recommendations.
With the client's written authorization, the report may be shared with their attorney, estate-planning professional, physician, or other designated professional.

Clarity provides clinical assessment and documentation—not legal advice or a legal determination of capacity.
This consultation does not provide legal opinions, forensic capacity determinations, testamentary capacity opinions, formal neuropsychological evaluation, or ongoing treatment.
When additional evaluation is appropriate, Clarity may recommend follow-up with a physician, neurologist, neuropsychologist, or other qualified professional.
