Depending on the clinical question, the consultation may include developmental history, standardized questionnaires, objective performance-based measures, clinical interview, relevant records or collateral information, individualized feedback, and a comprehensive written report.

Cognitive Baseline & Brain Health Evaluation
A structured assessment for adults and families seeking objective information about current cognitive functioning or nonurgent concerns involving memory, attention, processing speed, executive functioning, language, or changes in day-to-day functioning.
The evaluation may include relevant history, informant information when appropriate, objective cognitive measures, clinical consultation, individualized feedback, and a detailed written report.

Brain Track™ Annual Cognitive Monitoring
An optional $295/year monitoring service for eligible clients who have completed an initial Cognitive Baseline & Brain Health Evaluation.
Brain Track™ provides annual cognitive reassessment, comparison with established baseline information, longitudinal trend review, and an updated summary report. With authorization, findings may be coordinated with the client's physician or other designated healthcare professional.
Cognitive Evaluation for Estate Planning
A focused consultation for individuals, families, and professional representatives seeking objective information about cognitive functioning in connection with estate planning or related personal or legal considerations.
The consultation provides clinical assessment and professionally documented findings within Clarity's defined scope. It does not provide legal advice or make a legal determination of capacity.

Clarity's services are consultation-based. We do not provide emergency or crisis care, ongoing psychotherapy, medication management, controlled-substance prescribing, or formal neuropsychological evaluation.

Clarity is intentionally focused on structured adult cognitive and neurodevelopmental consultation.
Our telehealth model allows appropriate portions of the assessment process to be completed securely before your live clinical consultation. Depending on the service, this may include history forms, standardized questionnaires, collateral information, and objective performance-based cognitive measures.
Rather than relying on a single questionnaire, screening score, or cognitive task, we consider multiple sources of information, including:
Developmental, medical, psychiatric, and functional history
Current concerns and day-to-day experiences
Standardized assessment measures
Objective performance-based measures when clinically appropriate
Relevant records
Collateral information when available and authorized
Clinical interview and professional clinical judgment
We also recognize that adult autism and ADHD can present differently across individuals and environments. Masking, compensation, learned coping strategies, and differences in communication or social presentation may be important parts of the clinical history.
The result is a structured, individualized consultation that explains what the available information supports, what remains uncertain, and what may be appropriate to consider next.

We confirm your age, location, presenting concern, and whether the requested consultation falls within Clarity's scope.
02 — Scheduling & Intake
Before your appointment, you'll receive consent, financial, privacy, telehealth, and intake materials through our secure digital process.
03 — Online Assessment
You complete appropriate questionnaires and digital assessment measures from home before your clinical consultation. Collateral information may also be collected when relevant and authorized.
04 — Clinical Consultation
You meet with Dante' Peterson, FNP-C, PMHNP-BC, for a structured telehealth consultation focused on your history, concerns, functioning, and the question being evaluated.
05 — Feedback & Report
You'll receive individualized feedback and a professionally prepared written consultation report describing relevant history, findings, clinical impressions, limitations, and recommendations.
06 — Next Steps
When appropriate, recommendations may include follow-up with primary care, psychiatry, neurology, neuropsychology, psychotherapy, or another appropriate professional.
With your authorization, your report can also be shared with your physician, healthcare professional, attorney, estate-planning professional, or other designated professional.

When clinically appropriate, Clarity may use digital performance-based cognitive measures through Creyos.
These interactive, web-based tasks can provide objective information about areas such as attention, memory, processing speed, reasoning, and executive functioning.
The purpose is to add objective performance information to the broader clinical assessment. Results are interpreted alongside your history, clinical interview, standardized measures, functional information, and other available data rather than being treated as a diagnosis on their own.
Digital assessment also allows appropriate portions of the evaluation to be completed remotely, reducing the need for every component to take place during a live appointment.

Your consultation is directed, reviewed, and finalized by Dante' Peterson, FNP-C, PMHNP-BC.
Her clinical background spans both family practice and psychiatric mental health, allowing relevant medical, behavioral-health, developmental, and functional factors to be considered within the scope of the consultation.
Your selected service includes individualized feedback and a professionally prepared written report.
Depending on the consultation, the report may address relevant history, assessment findings, clinical impressions, functional considerations, limitations, and recommendations or next steps.
With your authorization, the report may also be shared with designated healthcare or professional contacts when appropriate.

This allows each consultation to have a defined scope, transparent fee, and clearly communicated process without structuring the service around insurance authorization requirements.
Clarity does not bill commercial insurance directly.

Insurance & Reimbursement
Clarity does not bill insurance directly. If your plan provides out-of-network benefits, you may choose to submit documentation independently. Reimbursement is determined by your individual insurance plan and is not guaranteed.
Applicable financial policies, payment terms, and Good Faith Estimate information are provided before services begin.

Timelines may be affected when required records, collateral information, or other assessment materials have not been received.
Report turnaround refers to the delivery of the written consultation report. It does not guarantee a particular diagnosis, clinical impression, or outcome.

Yes. With your authorization, a Clarity report can be shared with your physician, healthcare professional, school, university, employer, attorney, estate-planning professional, or another designated professional when appropriate.
A treating clinician may consider the report as part of their own clinical decision-making. Decisions regarding treatment, medication, additional testing, or other care remain with the appropriate treating professional.
Reports may also provide documentation of clinically relevant findings when pursuing workplace or educational accommodations. However, no evaluation can guarantee eligibility for accommodations.
Schools, universities, employers, licensing organizations, and testing bodies may have their own documentation requirements, including requirements regarding the type of professional who performs an evaluation.
If accommodations are an important reason for seeking an evaluation, we recommend reviewing the specific requirements of your institution before scheduling.

Yes. Clarity offers a Cognitive & Estate Planning Consultation for individuals, families, attorneys, and estate-planning professionals seeking objective information about cognitive functioning when it may be relevant to estate-planning matters.
The consultation is designed to provide a focused clinical assessment of relevant cognitive and functional factors, along with a professionally prepared written report documenting the information considered, assessment findings, clinical impressions, limitations, and recommendations.
With appropriate authorization, findings may be shared directly with an individual's attorney, estate-planning professional, physician, or other designated professional.
Clarity provides clinical assessment and documentation—not legal advice or a legal determination of capacity. Questions regarding the legal validity of documents, testamentary capacity, competency, or other legal standards remain within the appropriate legal and professional framework.
This consultation may be appropriate when an individual or professional representative wants additional objective information about cognitive functioning in connection with an important estate-planning decision.

When a question falls outside our scope, we may recommend an appropriate level or type of care, which could include primary care, psychiatry, neurology, formal neuropsychological evaluation, psychotherapy, occupational therapy, speech-language pathology, sleep evaluation, or another specialty service.
