THE REFERRAL WORKFLOW
Simple from referral to report.
01 — REFER
Submit a referral inquiry or direct the patient to Clarity Cognitive Consultants.
02 — SCREEN
The clinical question, age, location, and appropriateness for the requested consultation are reviewed.
When a request falls outside Clarity’s scope, the individual may be directed toward a more appropriate level or type of care when possible.
03 — INTAKE
The patient completes secure digital intake materials, questionnaires, and applicable assessment measures before the clinical consultation.
04 — CLINICAL CONSULTATION
A structured telehealth consultation is conducted with Dante’ Peterson, FNP-C, PMHNP-BC.
05 — CLINICAL SYNTHESIS
Relevant history, standardized assessment measures, objective performance-based information when appropriate, and available collateral or records are considered together as part of the clinical assessment.
06 — REPORT
A professionally prepared consultation report is completed according to the selected service and access option, including relevant findings, clinical impressions, limitations, and recommendations.
07 — SHARE FINDINGS
With appropriate patient authorization, the completed report and relevant findings may be shared with the referring physician, healthcare professional, attorney, estate-planning professional, or other designated professional.

Report timelines begin following completion of the clinical consultation and receipt of all required assessment materials.
Patients receive an itemized receipt and, when applicable, documentation they may submit independently to their insurance plan for consideration of out-of-network reimbursement.
Insurance reimbursement is not guaranteed and depends on the individual's insurance plan.

Clarity Cognitive Consultants operates as a focused consultation practice. Our defined scope allows us to concentrate on the clinical question at hand while supporting continuity with the professionals already involved in the individual's care or planning.
Clarity does not provide:
Ongoing psychotherapy
Long-term psychiatric treatment
Medication management
Controlled-substance prescribing
Emergency psychiatric stabilization
Acute crisis management
Longitudinal primary care
Legal opinions or legal advice
Legal or forensic determinations of capacity
Formal neuropsychological evaluation
When an assessment identifies a concern requiring ongoing treatment, additional medical investigation, or a different level of evaluation, appropriate next steps may include follow-up with the individual's established healthcare professional or referral to another qualified specialist.
With the individual's authorization, Clarity can communicate relevant findings and provide the completed consultation report to the referring clinician, physician, attorney, estate-planning professional, or other designated professional.

Clarity Cognitive Consultants is a specialized clinical consultation practice founded and led by Dante’ Peterson, FNP-C, PMHNP-BC, a Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner.
The practice's clinical approach brings together relevant medical, psychiatric, developmental, and functional perspectives when considering adult neurodevelopmental and cognitive concerns.
All consultations are provided within the scope of practice and applicable licensure requirements for the patient's location at the time of care.
