What this evaluation is—and isn't
This service provides a clinical cognitive assessment and screening, not a formal neuropsychological evaluation. It is not intended to diagnose dementia, replace neurological or medical evaluation, or independently establish legal capacity.
When clinically indicated, findings may help identify areas that warrant further evaluation or provide a useful baseline for future comparison.

An Estate-Planning Cognitive Evaluation does not determine testamentary capacity, legal competency, legal capacity, or the validity of a will, trust, power of attorney, or other legal instrument. It does not replace the advice or judgment of the client's attorney.
The purpose of the evaluation is to provide objective clinical findings that may appropriately inform the professional team's understanding of the individual's cognitive and mental-status presentation at the time of assessment.

Brain Track™ is an optional annual monitoring service available to eligible clients who have completed a qualifying cognitive evaluation with Clarity.
$295 annually
Each annual monitoring cycle includes:

Review of longitudinal trends
Brief clinical interpretation
Updated summary report
Optional authorized communication with the client's healthcare team
Brain Track™ is designed for longitudinal monitoring—not evaluation of a new or rapidly changing cognitive concern.
A new clinical evaluation may be recommended if significant symptoms or functional changes emerge.
What Your Fee Includes
Clarity's fees are designed to reflect the complete evaluation process rather than simply the time spent in a video appointment.
Your evaluation may involve:

Private-Pay & Out-of-Network
Clarity Cognitive Consultants is a private-pay, out-of-network practice. Payment is due according to the terms provided during scheduling.
Because services are provided outside of insurance networks, you are responsible for the full fee.
An itemized receipt or superbill may be provided when appropriate. Insurance reimbursement is not guaranteed and depends on your individual plan. Clarity does not submit claims to insurance on your behalf.

Before You Schedule
Clarity Cognitive Consultants is a focused evaluation practice rather than an ongoing treatment practice.
Services do not include:
Ongoing psychotherapy
Ongoing psychiatric medication management
Controlled-substance prescribing
Emergency or crisis services
Longitudinal primary care
Formal neuropsychological testing
Forensic evaluations
Legal or testamentary capacity determinations
If your needs fall outside the scope of a focused evaluation, you may be directed to an appropriate medical or behavioral-health resource.
Good Faith Estimate
Under applicable federal law, individuals who are uninsured or who do not plan to use insurance may be entitled to receive a Good Faith Estimate of expected charges before scheduled services.
Your evaluation fee will be disclosed before scheduling, and you may request additional information about expected costs at any time.
